Mons Pubis Liposuction in Seoul: Slimming a Full or Bulging Mons

Mons pubis liposuction removes excess fat from the mons pubis, the soft mound above the pubic bone, through one or two tiny incisions so the area sits flatter in underwear, swimwear and fitted clothing. It reduces volume only; it does not lift loose skin, which needs a monsplasty. At Somssi Women's Clinic in Gangnam, Seoul, Dr. Yoon assesses both fat and skin before recommending which procedure, or both, is right for you.

What is mons pubis liposuction?

The mons pubis is the rounded pad of fat that covers the pubic bone above the vulva. In some women it is naturally full; in others it becomes prominent with weight gain, pregnancy or hormonal change, and it is often one of the last areas to slim down with diet and exercise. Mons pubis liposuction removes some of that fat through tiny incisions so the mound sits flatter and blends with the lower abdomen.

Liposuction treats volume only. If the skin over the mons has stretched and now hangs, especially after major weight loss, removing fat alone can leave it looser. For that, the skin-lifting procedure called monsplasty is needed, sometimes together with liposuction. Deciding between the two is the main purpose of the consultation.

Somssi Women's Clinic is a surgical gynecology clinic in Gangnam, Seoul, founded in 2000 and led by Dr. Yoon Ho-joo, MD, PhD. Because the clinic treats the whole external genital area, including the labia majora and perineum, mons contouring is planned so that the result is in proportion with the rest of the vulva rather than treated as an isolated bulge.

Who is a good candidate?

  • A full or protruding mons pubis that shows through leggings, swimwear or fitted trousers
  • A pubic bulge that persists despite a healthy weight and exercise
  • Skin over the mons that is still reasonably firm and elastic, so it can retract after fat removal
  • A stable weight for several months before surgery
  • Good general health, non-smoker or willing to stop around surgery, not pregnant
  • Realistic expectations: a flatter, more proportionate mons rather than a completely flat one

If you have lost a great deal of weight and the mons has dropped so that it hangs over the vulva or the pubic crease, liposuction alone is unlikely to satisfy you, and Dr. Yoon will discuss monsplasty instead. Women whose main complaint is the labia rather than the mons are better served by labia majora procedures.

How Somssi performs mons pubis liposuction

Assessment

Dr. Yoon examines the mons standing and lying down, pinches the fat layer to estimate how much can be removed, and checks skin elasticity. Photographs are taken for planning. If the labia majora are also full or sagging, or if the labia are deflated and would benefit from fat transfer using the harvested fat, that is planned at the same time.

Anesthesia and tumescent infiltration

The area is numbed with a long-acting local anesthetic solution, and most patients also choose sedation so they have no memory of the procedure. The anesthetic solution also constricts small vessels, which keeps bruising and bleeding down.

Fat removal

Through one or two incisions a few millimeters long, placed in the bikini line or pubic hair, a fine cannula is passed back and forth to loosen and remove fat evenly across the mons. The goal is a smooth, slightly convex contour, not a hollow. Removing too much fat is a known mistake in this area and can leave the mons looking flat or uneven, so the clinic favors a conservative approach.

Closing

The small incisions are closed with a fine suture or left to close on their own, and a light compression garment or supportive underwear is applied to help the skin settle.

Anesthesia, duration and hospital stay

Mons liposuction is performed under sedation with long-acting local anesthetic, or under local anesthesia alone if you prefer. The procedure is short; Dr. Yoon will tell you the expected time for your case once he has seen how much fat is to be removed and whether other procedures are combined. As with all of the clinic's procedures, there is no hospital stay: you rest, pass urine and go home the same day.

Recovery timeline

When What you can do
Day of surgery Go home with a compression garment or firm underwear. Expect swelling, bruising and some fluid leakage from the incisions in the first 24 hours.
Days 1 to 3 Walk around the house and do light activity. Shower as instructed. Take the prescribed medication.
Week 1 Bruising is at its most visible and then fades. Return to desk work. Wear compression as advised.
Weeks 2 to 3 Swelling settles noticeably. Resume walking and light exercise. Numbness of the skin is common and temporary.
Weeks 4 to 6 Return to vigorous exercise, cycling and intercourse when Dr. Yoon confirms the area is comfortable.
Months 3 to 6 Final contour is visible as the last swelling and any firmness resolve.

Results and what to expect

Within a few weeks the mons looks flatter and the transition to the lower abdomen is smoother. The final result is judged at about three to six months, once the skin has retracted and the deep swelling is gone. Fat cells removed by liposuction do not grow back, but the remaining cells can enlarge with weight gain, so the result is durable only if your weight is stable.

Results vary with skin elasticity. Younger, firmer skin retracts well; thinner or stretched skin may not, which is why some women need a lift as well. Liposuction does not change the labia majora or the vaginal canal and does not affect sexual function. If you want the labia treated as well, that is planned as a separate step in the same session rather than assumed.

Risks and how we reduce them

  • Bruising, swelling and temporary numbness of the mons skin
  • Bleeding or a collection of blood or fluid (hematoma or seroma)
  • Infection at the incision sites
  • Contour irregularity, dents or asymmetry
  • Over-correction leaving a flat or hollow mons, or under-correction
  • Skin laxity that becomes more noticeable after fat is removed
  • Prolonged firmness or swelling, especially in the pubic area where fluid tends to pool
  • Reactions to anesthesia

We reduce these risks by removing fat conservatively and evenly with a fine cannula, by using a tumescent anesthetic that limits bleeding, by assessing skin quality honestly before surgery, by compression and early walking after surgery, and by follow-up that overseas patients can do by photo and message. What is the difference between mons pubis liposuction and monsplasty?

Liposuction removes fat and relies on your skin to shrink back. Monsplasty removes a strip of loose skin (and often some fat) and lifts the mons, leaving a scar hidden low on the abdomen. If your mons is full but the skin is firm, liposuction is enough. If the skin hangs, especially after weight loss, you need the lift. Many women have both in one session.

Will mons liposuction get rid of my "FUPA"?

If the bulge is mostly fat, liposuction can flatten it considerably. If part of the bulge is loose skin or a low-hanging mons, fat removal alone will not fix it and may make the skin look looser. An honest examination at consultation will tell you which applies to you.

How much fat can be removed?

Only a modest amount. The mons is a small area, and over-treating it causes dents, flatness and loose skin. The aim is a mons that is in proportion with the lower abdomen and labia, not a completely flat pubic area. Dr. Yoon will show you what is realistic for your anatomy.

Does the fat come back?

Removed fat cells do not return, but the cells that remain can enlarge with weight gain, and hormonal changes can also affect the area. A stable weight gives the most durable result. Pregnancy after surgery will change the area again.

Can I combine this with labia majora surgery?

Yes. A full mons and sagging or deflated labia majora often go together, and treating them in one session gives a more balanced result with one recovery. Fat removed from the mons can even be used for fat transfer to deflated labia majora if that is appropriate.

Will there be visible scars?

The incisions are a few millimeters long and placed in the bikini line or within the pubic hair, so they are rarely noticeable once healed. A monsplasty, by contrast, leaves a longer horizontal scar low on the abdomen, hidden by underwear.
